How the score is produced

Each answer is worth 0–4. The total is divided by the maximum and shown as a percentage, then placed in one of three bands at 34% and 67%. That arithmetic is fixed — the AI writes the words, never the number.

What it is not

Screening, not diagnosis. No single questionnaire can diagnose anything; a clinician takes history, context and time. If something serious shows up, the page says so and shows the crisis line before anything else.
